Abstract

¸ñÀû: ÀÌ ¿¬±¸ÀÇ ¸ñÀûÀº »ó¾Ç ¹× ÇϾÇÀÇ ¿ÏÀü ¹«Ä¡¾Ç ¹× ºÎºÐ ¹«Ä¡¾Ç ºÎÀ§¿¡ ½Ä¸³µÈ °ñÀ¯Âø¼º ÀÓÇöõÆ®ÀÇ º¸Ã¶ Àü Ãʱ⠻ýÁ¸À² ¹× »ýÁ¸°ú °ü·ÃµÈ ¿ä¼Ò¸¦ Æò°¡ÇÏ°í ½ÇÆи¦ ¾ß±âÇÏ´Â ¿äÀο¡ ´ëÇØ ¾Ë¾Æº¸´Â °ÍÀÌ´Ù.

¿¬±¸ Àç·á ¹× ¹æ¹ý: 2004³âºÎÅÍ 2013³â±îÁö 770¸íÀÇ È¯ÀÚ¿¡¼­ ÃÑ 2158°³ÀÇ °ñ³» ÀÓÇöõÆ®¸¦ ½Ä¸³ÇÏ¿´´Ù. ÀÓÇöõÆ®ÀÇ ¼Ò½Ç°ú ȯÀÚÀÇ ¿¬·É ¹× ¼ºº°, ½Ä¸³ À§Ä¡, ÀÓÇöõÆ® ½Ã½ºÅÛ, ±æÀÌ ¹× Á÷°æ, ±×¸®°í °ñÀÌ½Ä ¹æ¹ý°úÀÇ ¿¬°ü¼ºÀ» Æò°¡Çϱâ À§ÇØ ÀÓ»óÀû ºñ±³¸¦ ½ÃÇàÇÏ¿´´Ù.

°á°ú: ÀÓÇöõÆ® ½Ä¸³ À§Ä¡¿¡ µû¶ó »ó¾Ç ÀüÄ¡ºÎ¿¡¼­ 98.23%, »ó¾Ç ±¸Ä¡ºÎ 96.97%, ÇÏ¾Ç ÀüÄ¡ºÎ 97.85%, ÇÏ¾Ç ±¸Ä¡ºÎ 98.75%ÀÇ ÀÓÇöõÆ® »ýÁ¸À²À» º¸¿´´Ù(P < 0.05). ÀÓÇöõÆ®ÀÇ Á¾·ù ¹× Ç¥¸éó¸® ±â¹ýÀº ÀÓÇöõÆ® »ýÁ¸À²°ú ƯÀ̼ºÀ» º¸ÀÌÁö ¾Ê¾Ò´Ù. ÀÓÇöõÆ® ¸Å½ÄüÀÇ Á÷°æ¿¡ µû¶ó 3.0 mm ÀÌÇÏÀÇ ÀÓÇöõÆ®¿¡¼­ 100%, 3.0 - 3.5 mm 97.09%, 3.5 - 4.0 mm 98.19%, 4.0 - 4.5 mm 98.29%ÀÇ »ýÁ¸À²À» º¸¿´°í, 5.0 mm ÀÌ»óÀÇ Á÷°æÀ» °¡Áø ³ÐÀº Æø°æÀÇ ÀÓÇöõÆ®¿¡¼­ 75%ÀÇ ºñ±³Àû ³·Àº »ýÁ¸À²À» º¸¿´´Ù(P < 0.05). ÀÓÇöõÆ® ¸Å½ÄüÀÇ ±æÀÌ¿¡ µû¶ó 9 - 11 mm ¹× 11 - 13 mmÀÇ ±æÀ̸¦ °¡Áø ÀÓÇöõÆ®¿¡¼­ °¢°¢ 98.12% ¹× 98.17%ÀÇ ³ôÀº »ýÁ¸À²À» º¸¿´À¸³ª Åë°èÇÐÀû À¯ÀǼºÀ» °¡ÁöÁø ¸øÇÏ¿´´Ù. °ñÀ̽ļúÀÇ ¹æ¹ý¿¡ µû¶ó »ó¾Çµ¿ °ñÀ̽ļúÀ» ´Üµ¶À¸·Î ½ÃÇàÇÑ ºÎÀ§¿¡¼­ ÀÓÇöõÆ® »ýÁ¸À²Àº 89.05%, °ñÀ¯µµ Àç»ý¼úÀ» ´Üµ¶À¸·Î ½ÃÇàÇÑ °æ¿ì 98.28%, »ó¾Çµ¿ °ñÀ̽ļú°ú °ñÀ¯µµ Àç»ý¼úÀ» µ¿½Ã¿¡ ½ÃÇàÇÑ °æ¿ì 98.34%, °ñÀ̽ļúÀÌ µ¿¹ÝµÇÁö ¾ÊÀº °æ¿ì 99.28%ÀÇ »ýÁ¸À²À» º¸¿´´Ù(P < 0.05).

°á·Ð: À̹ø ¿¬±¸¿¡¼­ ÀÓÇöõÆ® ½Ä¸³ ÈÄ ÃÖÁ¾ º¸Ã¶¹° ½ÃÀû Àü Ãʱâ´Ü°è¿¡¼­ÀÇ ÀÓÇöõÆ® »ýÁ¸À²Àº ÀÓÇöõÆ®ÀÇ ½Ä¸³À§Ä¡, ÀÓÇöõÆ® ¸Å½ÄüÀÇ Á÷°æ, ±×¸®°í °ñÀÌ½Ä ¹æ¹ý°ú °ü·ÃÀÌ ÀÖ¾ú´Ù. ÀÌ·¯ÇÑ °á°ú¿¡ µû¶ó °¢°¢ »ó¾Ç ±¸Ä¡ºÎ, 5.0 mm ÀÌ»óÀÇ ³ÐÀº ÀÓÇöõÆ®, ±×¸®°í »ó¾Çµ¿ °ñÀ̽ļúÀ» ´Üµ¶À¸·Î ½ÃÇàÇÑ ºÎÀ§¿¡¼­ ÇöÀúÇÏ°Ô ³·Àº »ýÁ¸À²À» È®ÀÎÇÒ ¼ö ÀÖ¾ú´Ù.

Purpose: The aim of the study was to evaluate preprosthetic initial survival rate and factors associated with survival of osseointegrated implants placed in edentulous area of maxilla and mandible and to suspect the possible causes leading to failure.

Materials and Methods: A total of 2158 endosseous implants that had been inserted between 2004 through 2013 were placed in 770 patients. The clinical comparisons were performed to evaluate implant loss in relation to age and gender of patients, position, system, length and diameter of implant, and bone graft technique.

Results: According to position, the survival rates were 98.23% in maxillary anterior site, 96.98% in maxillary posterior site, 97.85% in mandibular anterior site and 98.76% in mandibular posterior site (P < 0.05). According to diameter of implant, the survival rates were 100% under 3.0 mm, 97.09% between 3.0 to 3.5 mm, 98.19% between 3.5 to 4.0 mm and 98.29% between 4.0 to 4.5 mm but relatively lower survival rate was 75% in 5.0 mm-over (P < 0.05). The survival rates of implants were 89.51%, 98.28%, 98.34% and 99.27% in the group with isolated sinus graft , with isolated GBR, with sinus graft and GBR simultaneously and without bone graft, especially (P < 0.05).

Conclusion: This study establishes a relationship between survival rate of implant and position, diameter of implant system and bone graft technique. In conclusion, there were low survival rates in maxillary posterior site, in dental implants with wide diameter of 5 mm-over, and in the group with isolated sinus graft.
